Forms
The forms I ask you to sign, so you can read them at home rather than for the first time on the table. Print one and bring it, or just come as you are — there are always paper copies in the room.
These forms are drafts and are not yet in force. They are published so they can be read. A form takes effect only when it is signed and dated.
- Intake, Consent & Liability Release Signed before every session. A new copy is signed at each visit.
- Records Authorization Only if you want notes shared with another provider involved in your care.
- Marketing Release Entirely optional. You do not need to sign this to book or be seen.
- Media Release Entirely optional. You do not need to sign this to book or be seen.
- Health History Filled in once, at your first visit. This is the long one — worth doing at home if you can.
- Before We Begin Today Asked at every visit, out loud, and I write the answers down. You do not need to fill this in beforehand — it is here so you can see what I ask.
If you would rather not print anything, that is completely fine. Nothing here needs to be done in advance, and I would rather you arrived without a form than not at all. If reading a screen is difficult, ring me and I will post one out or read it to you and write down what you say.
Every form shows a version and a date. If the copy you are holding is older than the one listed here, use this one.
